What is GAPP?

The Georgia Pediatric Program helps eligible Medicaid members under age 21 receive medically necessary skilled nursing and personal support services at home.

What do I need to make a referral?

Start with your contact information, county, your child’s age range and the type of support you are exploring. We will securely collect clinical details later.

Does submitting a referral guarantee approval?

No. GAPP eligibility and service hours are determined through the Georgia Medicaid review process based on eligibility and medical necessity.

Can a physician or case manager refer a child?

Yes. Families, physicians, hospital teams, case managers and community partners can start a referral.
